Vital Records

Certified copies of Missouri birth and death records can be obtained at the Shannon County Health Center’s main office at 18018 Grey Jones Drive in Eminence.  We can print birth records dating back to 1920 and death records dating back to 1980.

If you need documents prior to those dates, contact the Bureau of Vital Records in Jefferson City at 573-751-6387 or follow this link:  DHSS Vital Records

To get a copy at our office, you will need:

One primary identification document that includes your name and photo such as: a driver’s license, U.S. military ID card, U.S. passport, work or school ID, etc.

OR

At least two alternate forms of identification (Documents must contain your name and the name of the company or organization issuing the document.)

 AND

Payment by cash, check or money order.

Fees for records obtained at the Shannon County Health Center are:

Birth Records

$15 per record

$15 for each add’l copy

Death Records

$14 for first certified copy of record

$11 for each add’l copy of the same record ordered at the same time

Vital Record(s) Application:

You can come to our office and complete an application for a vital record and receive it within a few minutes, or you may choose to download and print the form and complete the process by mail.  When ordering a record by mail, the form must be notarized and accompanied by a check or money order.

Notice
We do not have access to marriage and divorce documents.  Contact your local county court house or the Bureau of Vital Records in Jefferson City to obtain those documents.
